Report a Code of Conduct Violation

Complete the following form to report a violation of the SAA Code of Conduct. Please provide as much detail as possible so that SAA can address the situation immediately and thoroughly. 

When you click “submit,” your report will be emailed directly to SAA Executive Director, who will handle the report in confidence with the SAA Executive Committee and Council.

Anyone may anonymously report a Code of Conduct violation via this form. However, the Executive Director appreciates having contact information so that she can follow up for more information as needed and/or confirm that the violation has been addressed.
