Session Participant Agreement Form

In 2014, CoSA, NAGARA, and SAA are lucky to have over 300 participants in this conference! However, small details like permission to publish a speaker's name and institution or whether a speaker agrees to be taped need to be collected from each person participating in an education session.

If you have just given consent to travel to DC and appear at the Marriott Wardman Park as a speaker, chair, or moderator on an education session (e.g., "Session 103"), your sesison chair or the Conference Office will ask you to complete this form:
