n. ~ A change to a document modifying the meaning or intent of its content.

(CJS, Records §23) The record cannot be altered so as to show anything not in the instrument recorded at the time of the original record.
(Skupsky and Manta–a 1994, p. 124) Alteration of records is analogous to the destruction of records in that the original informational content of the records is obliterated and rendered unavailable for future use.
